In general, Koto Ward is an old traditional neighborhood (although most things turned into ashes during WW2). Kiyosumi, Fukagawa, Monzen-Nakacho sounds very old-fashioned grannies' places to the local Japanese, although I personally think those areas are quite nice and relaxing neighborhoods good for a long-term stay.
